Permission Slips
Below are the relevant permission slips for the forseeable future. You can fill out the permission slips online and we will get notified that you have completed it or you can click on the Hardcopy Permission Slips button below, print out the permission slip and fill it out. We do not need both a hardcopy and an electronic copy.
Electronic Instructions: Make sure you understand the event or trip before filling out the form. Click on the Start Signing button for the permission slip you would like to fill out. That will pop up a box for you to insert your name and email address. This is used to send you a copy of what you filled out. The form will walk you through filling it out with a confirmation and legal statement at the end. If you need to fill out more than one permission slip, refresh the page and it will show the slip again.
Please do not fill these forms out yet as we only have 10 signed documents per month on our current Jotform subscription
